This innovation project is an effort to redesign the health care delivery system in Illinois for those who are enrolled in both the Medicare and Medicaid programs (i.e. dual eligibles). The goal is to improve health outcomes while reducing cost growth. Voluntary enrollment begins on January 1, 2014.
