While Medicare has historically paid physicians and other practitioners for care management services only when provided within face-to-face visits, the Centers for Medicare & Medicaid (CMS) recently finalized a new “Chronic Care Management” payment code that provides an opportunity for Medicare providers to be paid for providing non-visit based care management services. This new payment was finalized as part of Medicare updated payment policies and payment rates for services furnished under the CY2015 Medicare Physician Fee Schedule.
